We take the rights of the public’s access to public records seriously. Every reasonable effort is made to expedite all requests for disclosure of public records; however, due to personnel demands it is not always possible for CareerSource Polk to specify a specific delivery date. We respect all anonymous submissions; however, one method of contact (phone number or email address) is required to follow up on your request.

To assist us in complying with public records law and requests for public records, please follow the outlined public records procedure provided below:
Please provide your name, email address, mailing address, and telephone number so that we may contact you if we have any questions. Additional information that assists us in responding includes but is not limited to:
- The name of the person or entity about which you are requesting information. Please note that the CareerSource Polk can only respond with public records that involve CareerSource Polk.
- The address of the person or entity the type of practice/entity (for example a medical doctor)
- A clearly stated time period for which you are requesting records and the specific type of records you are requesting (for example the final order imposing discipline)
- Other information that you have that you feel would help us identify the documents you are seeking.
Fees:
15 cents per page (single-sided copies)
20 cents per page (double-sided copies)
All confidential information will be redacted from copies of public records, and any request that requires extensive use of information technology resources or clerical or supervisory assistance may be assessed an additional charge (special service fee) but you will be contacted prior to being charged this fee. When possible your request will be provided in the manner you requested (for example a paper copy, by email, or on a CD) however you will be contacted first as to any additional costs.
On occasion we may need to contact you about your public record request. To assist us in contacting you should we have a question, please provide your name, telephone number, email address, and mailing address. You are not required to provide the information but an online contact form is provided for your convenience below.
Under Florida law, e-mail addresses are public records. If you do not want your e-mail address released in response to a public records request, do not send electronic mail to this entity. Instead, contact this office by phone or in writing.
